Nine minutes with the diabetes nurse

This is the call I run most often in this circle and the one I believe in most. Twelve people, all with an appointment coming up, and the entire hour spent turning vague dread into three questions each. That is most of what peer support can honestly offer when the appointment is nine minutes long, and it is worth a great deal more than it sounds.

Where the room started

With a lot of anxiety and very little specificity. Carl had a review in a fortnight and could not say what he was worried about beyond a general sense that something would be wrong. Ravi, nine years into type 2, had a folder of printouts and no idea which of the lines on them mattered.

What I do here is refuse to interpret anybody's results and instead explain what the words mean, which is a different job and the only one I am willing to do in this role. Units, reference ranges, what a trend looks like against a single wobble.

What came up

Three recurring worries, all reasonable. Whether an improving A1c means everything is fine, which it does not, because A1c is one measure of one thing and Ravi's lipids had moved in the other direction while his A1c improved. What happens to other diabetes medications alongside this, which is a prescriber conversation every single time and never a room conversation, particularly where there is any hypoglycaemia risk. And feet, eyes and kidneys, which get forgotten in a conversation that has been captured by one number.

Carl's three questions, in the end, were about his feet, his blood pressure tablets and how often he should expect bloods. None of them were the thing he walked in worried about.

Where we left it

Twelve people with three written questions each, and an agreement to report back on what actually got answered. Nobody left with a plan I gave them. That is the rule in this circle and it is not negotiable.
