I hold the symptom circles, which means I have heard more about other people's digestion than almost anyone alive. I have made peace with it.

My one repeated message is that the standard step-up schedule is a schedule, not a law of nature, and that plenty of people do better on a slower one — and that this is a conversation to have with a prescriber rather than a decision to make in a forum. I am firm about that. I will happily talk for an hour about what "worse in the first three days after a step" feels like and what has helped people, and I will not tell you what to take.

The other thing I say often: some symptoms are the ordinary tax and some are a reason to stop and get seen today. Knowing the difference is the single most useful thing this circle teaches. Severe pain that wraps to your back and will not settle, vomiting you cannot break, signs of dehydration, anything neurological — those are not "let us try ginger". We keep a plain list and I point at it constantly.

I started on tirzepatide in 2023, went up too fast, had a fortnight I would not repeat, dropped back a step and have been fine for two years. That is the whole of my expertise.
